I understand and acknowledge staff do not administer any medication but will supervise participants capable of administering their own. In case of a life-threatening situation where the participant is unable to administer medication such as an epi-pen or asthma inhalers, staff will assist.

I understand that, with proper parental consent, children may self-administer prescribed medications or injections in accordance with physician orders. I understand that, Medications, Epi Pens, inhalers, and all other child administered medications must have the prescription label.

Mobius staff will administer ice packs and band aids. I further authorize Mobius staff to provide emergency or urgent medical care as they deem necessary for my child’s health and safety.

In the event of a medical emergency, I authorize any licensed physician, hospital, or other qualified health care provider to provide prompt medical treatment to my child, whether or not I am able to be contacted or informed at the time of treatment. If your child requires medical attention, you will be contacted immediately and will be responsible for an incurred medical expenses.

Camps fees are non-refundable but are transferable (to another child). Sick kiddo or need to cancel? Please call the admissions desk at 509.321.7121 and leave message and/or email info@mobiusspokane.org for a credit. Camps fees for cancellations cannot be refunded but can be credited towards Homeschool Explorations classes (based on availability) or the next camp session (based on availability.) To receive a credit, you must call in advance prior to your camp start time. No shows or cancellations received after the camp start time will not receive a credit. Credits will be noted in your account and never expire.

I hereby grant permission for my child to participate in all activities. In consideration of this registrant’s participation in Mobius' programs, I (the parent or guardian of registrant is under the age of 18, or from self if registrant is 18 or older) hereby release, waive and discharge Mobius, and all of its instructors, employees, directors, agents and volunteers from any and all liability to the registrant, and to all the registrant’s legal representatives and next of kin for damage and injury to the registrant or to any person or property arising out of participation in the program, whether on Mobius' premises or elsewhere. This agreement includes, but is not limited to, claims or demands on account of injury or damage caused or allegedly caused by the negligence of Mobius or any of the individuals listed above.
